FOR ASSOCIATE MEMBERS: What to do?
a.) Listen to a brief orientation scheduled at the Academy and in your own Agency. b.) Fill up the application Form. c.) Submit the Application Form to the Cooperative Assistance Office of PADPAO Academy for endorsement to your Agency. d.) The Membership and Miscellaneous Fees of Php150.00 should go with (attached with) the Application Forms. e.) The Member-Equity Fee of Php5,000.00 may be paid by installment and which can be arranged with your respective Agency. f.) Fill up an authorization form for the Agency Operators to deduct a certain amount from your salary for your monthly contribution.
